JOB DESCRIPTION
CTH I RESIDENTIAL SUPPORT PROVIDER
Qualifications:

High School diploma or GED. One year of experience in working with adults with mental retardation or developmental disabilities preferred

Must have a valid driver’s license, and a good driving record, as determined by Board policy

Must be supported by three (3) or more work-related references

Primary Responsibility:

To teach and train people with disabilities to manage all aspects of daily living to include personal, financial, household, medical, health, safety, and leisure management to ensure the consumers obtain maximum independence. Provide needed supports to achieve and maintain maximum independence. Ensure documentation is complete and accurate for all contacts made with consumers.

Specific Responsibilities:

  1. Responsibilities Related to Individuals receiving Supports

a. Provides teaching and training opportunities to individuals in all aspects of daily living to include, but not limited to; personal, financial, household, medical, health, safety and leisure management using task analysis, modeling techniques, verbal or visual instruction, one-on-one assistance and any other acceptable teaching or training method.
b. Demonstrates flexibility, creativity and commitment in delivering supports to people with disabilities.
c. Responsible for transporting and supervising consumers to and from work, scheduled activities, medical appointments and recreational outings by utilizing the agency vehicle or personal vehicle if necessary.
d. Responsible for assisting consumers in participating in social and recreational activities related to his/her personal areas of interest. Assist in obtaining pertinent schedules, activities, and locations of desired activities. Seek opportunities for group activities and implement plans to participate in group activities.
e. Participates in and instructs consumers on their options concerning activities and/or use of leisure time and provides appropriate guidance resulting from the consequences of his/her choices.
f. Responsible for monitoring the health, safety, and well being of each consumer to whom he/she provides supports and reporting any issues that directly affect any of these areas. This includes but is not limited to, the reporting of abuse and allegations of abuse utilizing your immediate chain of command.

g. Observes consumers and reports to management any physical, social, emotional, or behavioral changes.
h. Maintains consumers’ records and reports in a complete and accurate manner respecting the confidentiality of each consumer’s information.
i. Reviews all consumers files quarterly to ensure all records and reports are complete, accurate and properly filed.
j. Studies and becomes familiar with the current program objectives and supports for the consumers and aids them in achieving these objectives through teaching, modeling, and role-playing in a manner that promotes learning.
k. Maintains a friendly but professional relationship with each consumer under his/her care and models positive interactions between self and co-workers. Exhibits sound judgment and reasonable decision making skills in the performance of duties.
l. Assists with medication delivery to ensure each consumer is provided with only his/her medication. Reports any medication errors to the appropriate supervisor and completes appropriate documentation of any error.
m. Assists consumers as needed in area of nutrition, including grocery shopping, meal preparation and cleanup. Follows inventory, purchasing and meal preparation procedures as required.
n. Consistently addresses challenging behaviors proactively, respectfully and by avoiding the use of adverse or deprivation intervention techniques. Where these techniques are included in an approved support plan, ensures that implementation is as specified, works to find alternatives, and advocates for the eventual elimination of these techniques from the person’s plan.
o. Utilizes strategies identified on Personal Outcome Measures to ensure that each consumer’s choice, goals and desires are being recognized and respected.
p. Assist consumers to become as independent as possible in maintaining their living environment by teaching and training consumers in maintaining a household.
q. Assist consumers with maintaining the grounds of each residence to promote pride and respect for the consumers and the organization.
r. Helps consumers understand the need for appropriate grooming and dressing and provide assistance in these areas as needed.
s. Assists consumers in managing financial matters. Has knowledge and understanding of the management of personal funds policy in order to assist consumers accurately and within the guidelines of the policy.
t. Responds to phone calls and pages from consumers and TDC staff in a timely manner.
u. Ensures continued smooth operation of the day –to- day activities of the program in the absence of the Director of Supervised Living Supports.
v. Performs any other related duties as required and/or requested.

General Responsibilities

  1. Relationships and Contacts with Others
    a. Conducts self in a manner that serves as a positive role model to other staff, consumers receiving supports/services, their families, volunteers, businesses, other agencies, and the public.
    b. Maintains an appropriate and generally accepted level of professionalism in all interactions during working hours and while representing the agency.
    c. Develops and maintains a cooperative working relationship with all Aiken County Board of Disabilities/Tri Development Center personnel as well as the public, other agencies and businesses.
  2. Schedule and time Management
    a. Will be expected to work a flexible schedule with flexible shifts as scheduled Sunday through Saturday. Flexibility before and after duty and weekend hours may be required and will depend on schedule demands.
    b. Will effectively complete all assignments in a timely manner.
    c. Will primarily work independent of direct supervision with professionalism and commitment to effective completion of tasks and duties.
    d. Will utilize his/her time in a productive manner throughout the workday.
    e. Must be willing to accept other positions temporarily, when necessary.
    f. May be subject to falls, infectious diseases, odors, noises and physical injury due to behavior interventions.
    g. This position will be considered non-exempt and is therefore eligible for overtime compensation.
  3. Communication
    a. Prepares daily and monthly documentation as required.
    b. Communicates in a manner that is easily understood, concise, and professional.
    c. Communicates activities and concerns to the immediate supervisor in order to ensure acceptable service delivery for all consumers served.
    d. Attends meetings relating to the consumers and/or program as required and/or directed.
    e. Continuously advocates for the needs of the consumers.
  4. Policies and Procedures
    a. Adheres to Aiken County Board of Disabilities/Tri Development Center Policies and Procedures as outlined in the policy manual.
    b. Follows the provisions of all memorandums, directives and applicable manuals.
    c. Follows all policies and laws pertaining to consumer confidentiality.
  5. Complete other duties as assigned.

Supervisor

The direct supervisor of the CTH I RSP will be the CTH I Coordinator.
a. Meetings will be held as necessary with his/her supervisor.
b. CTH-I RSP staff is responsible for keeping his/her supervisor informed of any problems related to performing his/her duties.
c. Adheres to the standards and policies under which the agency operates at all times.

Typical Physical Demands

The CTH-I RSP employee must have the ability to move intermittently throughout the workday. Activities include sitting, standing, bending, lifting, driving, using a keyboard, making and
transferring phone calls and taking messages, and possibly physical intervention with upset, combative, and/or psychotic consumers. He/she is subject to lifting, carrying, and restraining consumers and may be required to lift up to 50 pounds.
